An 18 handicap plays to a target score of 90 on a par-72 course of standard difficulty (Course Rating 72.0, Slope Rating 113) — and in most rounds shoots 92 to 95. Playing exactly to your handicap happens in roughly 20–25 % of rounds, not most of them.
What an 18 handicap shoots on different courses
A Handicap Index of 18.0 is not 18 strokes everywhere. The strokes you actually receive — your Course Handicap — depend on the Course Rating and Slope Rating of the tees you play:

Target score is Course Handicap + par: the score you shoot when you play exactly to your handicap. The typical range adds the 2–5 strokes a player is above their handicap in most rounds.

Where an 18 handicap gets strokes — and the maximum hole score
Handicap strokes are taken in stroke index order — the "handicap" row on the scorecard, which ranks the holes by difficulty. With a Course Handicap of 18 you receive exactly one stroke on every hole — the one handicap where the scorecard's ranking of the holes stops mattering. That allocation also caps each hole score you can post: the maximum for handicap purposes is a net double bogey — par + 2 + the strokes you receive on that hole (Rule 3.1b).

Net double bogey adjustments use your full, unrounded-then-rounded Course Handicap — here taken as 18, the Course Handicap on tees of standard difficulty. On other tees the allocation shifts with your Course Handicap (see the slope table above). Before a Handicap Index is established — your first three scores — the maximum hole score is par + 5 instead. If you post hole-by-hole in a scoring app, these caps are applied automatically.
How an 18 handicap is calculated
To carry a Handicap Index of 18.0, the average of your 8 lowest Score Differentials from your last 20 rounds needs to come out at 18.0. Each differential is (113 ÷ Slope Rating) × (Adjusted Gross Score − Course Rating), so on a standard course those eight best rounds average about 90.
Note that only your best 8 of 20 count. Your handicap describes what you are capable of, not what you average — which is why your typical round sits above your target score rather than on it.
